HealthCare for the Homeless (HCH) provides temporary medical services and case management to homeless individuals and families.
Visits by appointment or walk-in.

Monday through Friday

Please call the Cooperative Health Center (CHC) at 443-2584. You will be connected to the CHC clinic staff, or if you call after the CHC’s hours, you will reach an answering service that will connect you to the health provider that is covering on-call.
